4.      Professional Tax and Sales Tax Clearance Certificate.

5.      Domicile or Identification Certificate.

6.      Birth Certificate or School Leaving Certificate.

7.      N.O.C. from D.E. or Original Class IV Enlistment Certificate Enclosed.

8.      Attested Pass Photo (One).

9.      Panchayat Recommendation.

10.  All Photocopy attested by Gazetted Officer.
